Job Description

To provide high-quality patient care and triage services, assessing patient needs and prioritizing care accordingly. Collaborate with the senior clinical team to oversee remote monitoring systems and contribute to the delivery of excellent healthcare standards.

Key Responsibilities:

  • Assess patient needs and determine priority of care
  • Process referrals and contribute to the delivery of high standards of healthcare
  • Liaise with clinical teams in the community and integrated team to escalate concerns
  • Assess, prescribe, and arrange care interventions and assessments for patients referred to the Single Point of Access
  • Work within the integrated team to prevent unnecessary hospital admissions

Requirements:

  • Registered Adult Nurse Qualification
  • Diploma, BSc, or higher
  • Current NMC registration
  • Evidence of continuing professional development
  • Experience in working within a multidisciplinary team
  • Experience of working with patients with multiple pathologies and delivering care plans

Desirable:

  • Experience of working within a community setting
  • Knowledge of SystmOne
  • Experience of working within an acute or pre-hospital setting

  • Self-awareness
  • Enthusiastic and able to work under pressure

Working Conditions:

  • Be able to kneel, bend, and stoop and work in cramped environments
  • Able to manoeuvre limbs of up to 5kg-6kg
  • Able to manoeuvre patients using handling aids
  • Must have full UK driving licence and have access to a vehicle
  • Be able to make own travel arrangements to patients, clinics, base, and meetings as required
