**Job Overview**
The Frailty Hospital at Home team is part of Buckinghamshire Healthcare NHS Trust, an integrated provider of acute hospital and community services. As a Community Healthcare Specialist, you will play a vital role in delivering high-quality patient care in patients' homes.
Your responsibilities will include providing intensive, hospital-level care for acute conditions, managing complex medical needs, and working collaboratively with multidisciplinary teams. **Key Skills and Qualifications:**
- Degree/Diploma in Physiotherapy, Occupational Therapy, Nursing, or Paramedic Science
- Recognised teaching or mentorship qualification equivalent to level 3
- Experience of working as an advanced clinical practitioner/case manager
- Strong analytical and judgment skills, with ability to interpret complex situations
