About the Role
We are seeking a highly organized and detail-oriented individual to join our Ophthalmology Directorate as a Patient Pathway Coordinator. The successful candidate will have experience working in a healthcare setting and a solid understanding of patient pathways.
Key Responsibilities
- Ensure accurate and comprehensive patient records and clinical details are added onto the Electronic Patient Record (EPR) in a timely manner.
- Log and book appointments on EPR and assist the Patient Pathway Coordinators to ensure all referrals are clinically triaged within 48 hours of receipt of referral.
- Proactively monitor all outpatient clinics and highlight over or under booking of clinics to the Patient Pathway Coordinators.
- Ensure that patients on the Electronic Referral System (ERS) are given appointment dates in accordance with clinical priority and then length of wait.
- Assist the Patient Pathway Coordinators with managing and maintaining the appointments for booking list on ERS.
- Ensure that the appropriate action is taken following both patient and hospital cancellations, such that the utilisation of elective outpatient capacity is maximised and patients are kept informed.
- Ensure that the patients 18 week RTT pathway is correct on EPR before undertaking any changes to the patient pathway.
Person Specification
Essential
- GCSE or equivalent in English and Maths, grades A-C
- Good knowledge of patient pathways
- Maintains high standards and quality of work
Desirable
- Willingness to develop computer technology skills
Interpersonal Skills / Communication Skills
- Accurate data entry, typing and checking skills
- Excellent literacy, numeracy and communication skills both verbally and in writing
- Highly diplomatic and professional
- Positive, enthusiastic attitude
