About the Role

We are seeking a dedicated and compassionate Support Worker to join our team at Evolve Housing + Support. As a Support Worker, you will play a vital role in providing person-centred support to vulnerable individuals, helping them to regain their independence and achieve their goals.

Key Responsibilities
  • Identify and support customers' strengths and needs, developing personalized plans to achieve their goals and aspirations.
  • Work collaboratively with customers, colleagues, and external agencies to provide a comprehensive support service.
  • Contribute to a dynamic and supportive team environment, promoting a culture of equality, diversity, and inclusion.
Requirements
  • Knowledge of safeguarding principles and practices, with the ability to apply them in a day-to-day work setting.
  • Understanding of the reasons why people become homeless and the needs and challenges they face.
  • Ability to work shifts on a pre-planned rolling rota, including mornings, evenings, and weekends.
