About the Role
Hammersmith & Fulham Council is seeking a highly motivated and qualified Social Worker to join their Transitions Team. The successful candidate will work with young people and their families to effectively plan for adulthood, ensuring they are supported to navigate the complex route into Adult Services.
The role will require creative, community-based planning, working within statutory and local guidelines, and having a strong understanding of Safeguarding. The team will offer both planned and reactive support to this age group, and the post-holder will be responsible for a caseload of planned transitions and reactive casework.
The Council has developed the scope of its Transitions Team, and this is an exciting time to join the team. The successful candidate will be able to work flexibly whilst new policies and procedures are embedded into business as usual.
