Social Worker

Hammersmith & Fulham Council is seeking a skilled Social Worker to join our 3B Hospital Social Work Team. As a key member of our team, you will play a vital role in providing assessment and discharge support to residents of the 3 Boroughs: Westminster, Royal Borough of Kensington and Chelsea, and London Borough of Hammersmith and Fulham.
The successful candidate will work closely with our partners, including NHS/Health Colleagues, Community Independence Service, local CCG's, Continuing Health Care Teams, CLCH and CNWL Home First Teams, to ensure safe and timely discharges. You will hold a caseload of complex discharges, providing supervision to student social workers and ensuring excellent standards of Social Work practice underpinned by the Care Act 2014 and the Mental Capacity Act 2005.
Key Responsibilities- Completing care act assessments and facilitating safe discharges
- Partnership working with our partners to ensure effective discharge planning
- Managing a caseload of complex discharges, including supporting the senior practitioner and the Team Manager
- Providing supervision to student social workers
- Ensuring excellent standards of Social Work practice
