**About the Role**
We are seeking a skilled and compassionate Social Worker to join our Hospital Social Work Team in a busy and challenging role. As a Care Act Assessor, you will be responsible for completing thorough assessments and facilitating safe discharges for residents of the 3 Boroughs: Westminster, Royal Borough of Kensington and Chelsea, and London Borough of Hammersmith and Fulham.
Working closely with the management team, you will hold a caseload of complex discharges, providing supervision to student social workers and ensuring excellent standards of Social Work practice underpinned by the Care Act 2014 and the Mental Capacity Act 2005.
You will be an integral part of a collaborative and effective management team, working closely with NHS/Health Colleagues, Community Independence Service, local CCGs, Continuing Health Care Teams, CLCH, and CNWL Home First Teams.
**Key Responsibilities**
- Complete thorough assessments and facilitate safe discharges for residents
- Hold a caseload of complex discharges
- Provide supervision to student social workers
- Ensure excellent standards of Social Work practice
