Social Worker

We are seeking an experienced and skilled Social Worker to join our 3B Hospital Social Work Team. The successful candidate will be responsible for completing care act assessments, facilitating safe discharges, and partnership working with our partners. With a strong focus on quality of practice and continuous improvement, our team is dedicated to providing excellent care for vulnerable adults.
The ideal candidate will have excellent communication and interpersonal skills, with the ability to work collaboratively within a fast-paced environment. A strong understanding of the Care Act 2014 and the Mental Capacity Act 2005 is essential, as well as a commitment to upholding the highest standards of Social Work practice.
