Job Title: Health and Wellbeing Specialist

We are seeking a highly skilled and experienced Health and Wellbeing Specialist to join our team at Practice Plus Group, working in HMP Winchester.

This is a unique opportunity to work within the healthcare team, delivering high-quality care and support to patients with complex needs.

About the Role:
  • To provide 1-1 and group sessions, assisting the healthcare team on the wings and in-house blocks.
  • To deliver training and education sessions on substance misuse, healthy living, and related topics.

You will work closely with other healthcare professionals to ensure comprehensive care and treatment plans for patients.

Responsibilities:
  • Deliver high-quality care and support to patients, taking into account their individual needs and circumstances.
  • Collaborate with other healthcare professionals to ensure comprehensive care and treatment plans.
Requirements:
  • Experience of working within the substance misuse multi-disciplinary framework, including housing, social care, health care, and mental health pathways.
  • Excellent communication and interpersonal skills, with the ability to build rapport with patients from diverse backgrounds.
