About the Role
We are seeking a skilled and compassionate Healthcare Assistant to join our Urgent Care Service in Folkestone. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality patient care in the community.
Main Responsibilities
- Conduct comprehensive frailty assessments and develop personalized care plans in collaboration with patients and their families.
- Work closely with our team to ensure seamless care delivery and support junior colleagues in their clinical development.
- Drive between patient homes and work sites, utilizing your own vehicle.
Requirements
- Registered Healthcare Professional with a valid UK NMC PIN.
- Experience in recent community or acute healthcare, gained through education or work experience placement or previous employment.
- Awareness of professional issues and recent developments in the NHS, with knowledge of up-to-date clinical practice.
