Job Overview

We are seeking a dedicated and compassionate Healthcare Assistant to join our Urgent Care Service team in Folkestone. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality patient care in the community.

Main Duties
  1. Conduct comprehensive frailty assessments and develop personalized care plans in collaboration with patients and their families.
  2. Work closely with our team to ensure seamless care delivery and effective communication with patients, families, and healthcare professionals.
  3. Support junior colleagues in their clinical development and provide guidance and mentorship as needed.
  4. Drive between patient homes and work sites, utilizing your own vehicle.

Person Specification
  • Registered Healthcare Professional with relevant professional body (e.g., NMC, HCPC).
  • Valid UK NMC PIN.
  • Experience of recent community/acute healthcare gained through education or work experience placement or previous employment.
  • Awareness of professional issues and recent developments in the NHS.
  • Knowledge of up-to-date clinical practice.
  • Sound communication and interpersonal skills (verbal and written).
  • Ability to organize own work, cope with change, and use own initiative.
  • Current UK driving license and access to vehicle.
