About the Role

This Social Care Professional - Agency Leaving Care position at We Do Social Work requires a skilled and compassionate individual to provide support to young people transitioning out of care. The successful candidate will engage with young people, undertake needs assessments, and develop personalized plans to promote their independence and success.

Responsibilities and Duties
  • Engage with young people leaving care and build trusting relationships.
  • Conduct thorough needs assessments to identify areas for support.
  • Develop and implement individualized plans to promote young people's independence and success.
  • Maintain accurate records of interactions and progress using our computerized contact management system.
What We Offer

We Do Social Work provides a competitive hourly rate of £16.01, paid twice weekly through our Faster Pay service. You will also benefit from our free DBS and compliance service, dedicated consultant support, and access to a broad range of social work positions nationwide.

Requirements
  • Proven experience working with vulnerable populations, especially young people in care.
  • Exceptional communication and interpersonal skills.
  • Able to maintain confidentiality and handle sensitive information with discretion.
