Job Title: Social Worker - CAD TeamAbout the RoleWe are seeking an experienced and passionate Social Worker to join our CAD Team. This role is critical to ensuring the safety and wellbeing of our service users. Your primary responsibilities will include:
- Rapidly assessing referrals sent to Bury Adult Care Services to identify potential risks and provide appropriate support.
- Establishing regular contact with service users who meet the criteria for CAD Triage to ensure their safety and wellbeing.
- Liaising with various professional services, including CMHT, OPCMHT, Police, Hospital, and GP's, to develop comprehensive support plans.
- Identifying and accessing relevant services, such as Food Banks, BAME Groups, Men/Women/LGBT+ Support Groups, to enhance service user outcomes.
- Conducting initial Needs Assessments to facilitate the placement of service users in suitable housing.
- Carrying out home visits where necessary to assess service user needs and provide targeted support.
- Recognizing and responding to risk situations, taking prompt action to mitigate potential harm.
- Supporting Enquiry Officers by providing guidance on CAD Triage procedures and signposting service users to relevant teams.
- Carrying out further enquiries or signposting service users to relevant teams in response to EDT Notifications.
- Returning SF1 forms that do not meet SG Team criteria to CAD Triage for management.
To be successful in this role, you will need to possess a degree in Social Work and have experience working in adult social care. You must be passionate about making a positive difference in the lives of vulnerable adults.
