About the Position

We are seeking a highly skilled and compassionate Healthcare Professional - Community Care to join our team in Barnet.

This is an exciting opportunity for a Registered Nurse to develop their community nursing skills and be part of a dynamic integrated care team.

The successful candidate will work closely with the team leads and clinicians to develop the service, maintain close working relationships with both acute and community services, and provide holistic assessments of patients' health needs using a person-centred approach.

Main Responsibilities
  • Deliver high-quality community nursing care to patients in their homes.
  • Assess patients' health needs and develop effective care plans.
  • Collaborate with other healthcare professionals to ensure seamless care transitions.

Educational Requirements:

  • Registered Nurse with current UK NMC registration.
  • Evidence of post-registration education and training.

Work Experience:

  • Recent clinical nursing experience in a community setting.
  • Experience working with patients with complex health needs.
  • Supporting students and other learners.

Salary and Benefits: The estimated salary for this role is £35,000 - £55,000 per annum, depending on experience. We offer a comprehensive induction program, car lease scheme (terms and conditions apply), flexible working options, and opportunities for training and development.
