About the Role

Southern Health NHS Foundation Trust is seeking a highly skilled and dedicated Learning Disability Staff Nurse to join our North and West Hampshire Community Learning Disability Service. As a key member of our team, you will play a vital role in providing high-quality care to individuals with learning disabilities, working closely with our multidisciplinary team to deliver person-centered care.

Main Responsibilities

  • Assess, plan, implement, and evaluate the nursing care needs of a specific group of patients, working within the framework of MDT principles, the holistic model of care, and the six areas of clinical practice.
  • Supervise, mentor, and teach qualified and unqualified staff, including students, to ensure the development of a skilled and confident team.
  • Co-ordinate care for a specified group of patients, working closely with community, primary, secondary, and tertiary services to meet individual needs and service development.
  • Manage a caseload of patients under indirect supervision, being accountable for assessing, interpreting, planning, and implementing nursing treatment and care to patients within professional guidelines.
  • Ensure that systems are used to incorporate any adopted models of quality assurance and governance into everyday practice.
