Job Description

We are seeking an experienced Children's Nurse to join our evolving Children's Hospital @ Home team. The successful candidate will work collaboratively with the medical team at University Hospital Southampton NHS trust and GPs in the community to provide high-quality nursing care, support, and coordination of care for children and young people with short-term acute healthcare needs.

Role Overview

This is a fixed-term contract within the Children's Hospital @ Home service, which aims to provide early, safe planned discharge from hospital and prevent unnecessary admissions. As a Community Healthcare Specialist Nurse, you will be responsible for delivering specialist nursing care, support, and coordination of care to patients in their homes, via telephone calls, video calls, or home visits.

Main Responsibilities
  • To provide high-quality nursing care, support, and coordination of care to patients in their homes
  • To work collaboratively with the medical team at University Hospital Southampton NHS trust and GPs in the community to ensure seamless care across the region
  • To develop and maintain strong relationships with referrers, including GP Surgeries, Children's Emergency Department, Paediatric Short Stay Unit, and Paediatric Wards
  • To participate in regular supervisions, 1:1s, and appraisals to ensure ongoing professional development and support
Requirements

The ideal candidate will have:

A Registered Nurse (Children) qualification and current NMC registrationProven experience working in a community setting, preferably with children and young peopleStrong communication and interpersonal skills, with the ability to build effective relationships with patients, families, and healthcare professionalsA sound knowledge base of safeguarding principles and practicesBenefits
