Job Overview

We are seeking an experienced Children's Community Nurse to join our team as a Senior Community Children's Nurse. This role will involve providing specialist nursing care to children and young people with complex health needs, working collaboratively with families, schools, and other healthcare professionals to ensure seamless care delivery.

Key Responsibilities
  • Manage a caseload of patients with acute and short-term conditions, long-term conditions, disabilities, complex conditions, and those requiring continuing care.
  • Develop and implement individualised programmes of care for patients with complex health needs, including palliative care and life-limiting illnesses.
  • Support the management of integrated acute and community care pathways for children with short-term conditions.
  • Provide supervision and monitoring of junior staff.
  • Work with families to provide emotional support and specialist clinical care, sensitive to their changing and ongoing needs.
Requirements

To be successful in this role, you will need:

  • A BSc Nursing or RN in Child qualification and current registration with the NMC.
  • Post-registration specialist training relevant to the speciality.
