About the Role
Southern Health NHS Foundation Trust is seeking a full-time Band 5 Learning Disability Staff Nurse to work in the North and West Hampshire Community Learning Disability Service.
This is an exciting opportunity to join a busy and well-established multi-disciplinary health team supporting the healthcare needs of people with a learning disability throughout the North and West Hampshire Locality.
As a Learning Disability Staff Nurse, you will play a vital role as part of the community nurse team in a forward-thinking and innovative team.
We offer regular team days and nurse meetings, supervision, and opportunities for professional development, including leading and participating in local and divisional quality improvement initiatives.
Main Responsibilities
- Assess, plan, implement, and evaluate the nursing care needs of a specific group of patients.
- Work under the direction and clinical supervision of a nominated preceptor during a period of preceptorship.
- Supervise, mentor, and teach qualified and unqualified staff, including students.
- Co-ordinate care for a specified group of patients.
- Be a member of an integrated inter-agency, multi-disciplinary learning disability team.

Southern Health is one of the largest NHS Foundation Trusts in the UK, specialising in mental health and learning disabilities, as well as offering physical health community-based services.
We are currently working closely with other NHS Trusts to integrate all our collective community, mental health, and learning disability services.
Our new organisation, to be known as Hampshire and Isle of Wight NHS Foundation Trust, is set to launch in July 2024.
