Band 6 Dementia Care Lead

We are seeking a highly skilled and experienced nurse to join our team as an Advanced Clinical Practitioner - Dementia Care. The successful candidate will be responsible for providing advanced clinical practice and upskilling of frontline staff to support individuals with dementia.
Key Responsibilities- Provide education and support to individuals with dementia and their families to promote self-care strategies and independence.
- Develop multi-disciplinary informed personalised treatment and management plans for patients' health conditions.
- Complete comprehensive holistic assessments using clinical skills and knowledge to interpret complex clinical findings.
West London NHS Trust is an established partner in the development of the evolving North West London Integrated Care System and the Integrated Care Board. Our high secure services care for patients from South of England, and we provide low and medium secure services across eight London boroughs.
Salary and BenefitsThe estimated salary for this role is £44,964 - £54,961 per annum, depending on experience. In addition to a competitive salary, we offer a range of benefits including pension scheme, annual leave, and access to training and development opportunities.
