This is an exciting opportunity for a qualified nurse to take on a leadership role in our dementia care service at West London NHS Trust. As a Band 6 Dementia Care Lead, you will be responsible for developing and implementing high-quality care plans for individuals with dementia.
Key Responsibilities- Lead a team of nurses in providing education and support to individuals with dementia and their families.
- Develop and implement policies and procedures to improve the quality of care for patients with dementia.
- Collaborate with other healthcare professionals to develop multi-disciplinary informed personalised treatment and management plans for patients' health conditions.
