Job Summary:

This role is an exciting opportunity to join the Royal Devon University Healthcare NHS Foundation Trust as a Community Health Specialist. As a member of our team, you will be responsible for supporting the discharge process and facilitating timely hospital discharge to benefit our local community.

The successful candidate will require a driving licence and access to a car to complete community visits. A full-time position is available, but we are also open to discussing part-time hours. We offer a supportive environment with regular supervision, CPD groups, training, and peer support. You will also have access to a Champions Network that links you with therapists who are keen to share their knowledge of special interests.

A key aspect of this role is coordinating the smooth running of the service, which requires excellent IT skills. You will utilize our online patient record system, MyCare, to ensure seamless communication and coordination between teams.

The salary for this role is £35,000 - £42,000 per annum, depending on experience. Please note that preference will be given to staff with Priority and 'At Risk' status, including NHS At Risk staff throughout Devon.
