Learning Disabilities Social Worker Specialist

**Job Summary**This exciting opportunity has arisen for a highly motivated and enthusiastic social worker to join our Learning Disabilities Service. As a key member of the team, you will be responsible for providing high-quality support to individuals with learning disabilities, promoting their independence, choice, and control over their lives.
The successful candidate will work collaboratively with colleagues in health, commissioning, safeguarding, and other partner agencies to create innovative care plans, manage risk, and support people to achieve their goals and outcomes.
**Key Responsibilities**- Assess and develop individualised support plans that promote independence, choice, and control
- Work closely with colleagues in health, commissioning, safeguarding, and other partner agencies to deliver person-centred care
- Manage risk and ensure the safety and well-being of service users
