About the Role

We are seeking a highly motivated and compassionate Community Outreach Support Worker to join our team at Longhurst Group. As a key member of our Floating Support team, you will play a vital role in providing a comprehensive range of support services to customers with profound and enduring Mental Health issues and customers with Learning Disabilities, alongside those with a dual diagnosis.

Key Responsibilities
  • Maximising Independence: Support customers to maximise their independence and enable real choices in how they lead their lives.
  • Community Engagement: Support customers to engage and participate in the local community, as well as obtaining and maintaining settled accommodation.
  • Economic Wellbeing: Provide support to ensure strong economic wellbeing, such as helping customers to access their benefits entitlements, pay their rent and household bills, support with debts, etc.
  • Independent Living: Assist, support and prompt customers to live independently.
  • Record Keeping: Maintain accurate records of support provided, maintaining risk assessment and support plans.
  • Partner Agency Collaboration: Work with partner agencies such as Birmingham City Council and localised NHS services.
Requirements
  • Experience: Ideally have worked in a similar role supporting individuals with enduring mental health needs, although we're open to candidates who're interested in pursuing a new career within a Mental Health and Learning Disabilities environment.
  • Knowledge and Understanding: Having some knowledge and understanding of Mental Health and Learning Disabilities and the issues around it would be beneficial.
  • Communication Skills: Be comfortable in challenging negative beliefs and provide positive behaviour support, and always demonstrate a supportive and caring nature.
  • Driver's Licence: Hold a full driver's licence and access to a vehicle to travel to our customers across Birmingham in order to carry out your role.
