Job Title: Community Outreach Support Worker

We are seeking a highly motivated and compassionate Community Outreach Support Worker to join our team at IRIS - Networx Services. As a Community Outreach Support Worker, you will play a vital role in supporting individuals with mental health needs and learning disabilities in the community.

Key Responsibilities:
  • Maximise the independence of individuals requiring support and enable real choices in how they lead their lives.
  • Support customers to engage and participate in the local community, as well as obtaining and maintaining settled accommodation.
  • Provide support to ensure strong economic wellbeing, such as helping customers to access their benefits entitlements, pay their rent and household bills, support with debts, etc.
  • Assist, support and prompt customers to live independently.
  • Maintain accurate records of support provided, maintaining risk assessment and support plans.
  • Work with partner agencies such as Birmingham City Council and localised NHS services.
Requirements:
  • Able to work in a community setting, supporting individuals with mental health needs and learning disabilities.
  • Good communication and listening skills, with a strong ability to provide advice to help customers.
  • Comfortable in challenging negative beliefs and providing positive behaviour support.
  • Ability to work independently and as part of a team.
  • Hold a full driver's licence and access to a vehicle to travel to customers across Birmingham.
