Job Description

Superior Healthcare is a leading provider of healthcare services, dedicated to delivering high-quality care and support to individuals with learning disabilities and autism. We are currently seeking a skilled and compassionate Care Coordinator to join our team in Hailsham.

As a Care Coordinator, you will play a vital role in supporting individuals in care and supported living settings. You will work collaboratively with a multidisciplinary team to ensure that clients receive tailored care plans, promoting their dignity, respect, and overall well-being.

We offer a competitive salary of £22,000 - £28,000 per annum, depending on experience, plus a range of benefits including flexible shift patterns, ongoing training, and access to a comprehensive wellbeing program. Key Responsibilities:
  • SUPPORT INDIVIDUALS WITH LEARNING DISABILITIES AND AUTISM IN CARE AND SUPPORTED LIVING SETTINGS
  • WORK COLLABORATIVELY WITH MULTIDISCIPLINARY TEAMS TO DEVELOP AND IMPLEMENT PERSON-CENTERED CARE PLANS
  • Maintain accurate records and reports, ensuring compliance with regulatory requirements
Requirements:
  • REQUIRE UK DRIVER'S LICENCE AND ACCESS TO YOUR OWN VEHICLE
  • A minimum of 6 months' experience working in care settings or related fields
  • CARING, EMPATHETIC, AND PATIENT ATTITUDE WITH STRONG COMMUNICATION SKILLS
