Job Overview

The Access Team is a dynamic and fast-paced service that provides a single point of entry and first-level gatekeeping into mental health services in Stockport. Our team screens and triages all referrals, ensuring individuals receive the right care at the right time.

Main Duties
  • Conduct comprehensive assessments of individuals' health and social care needs, including risk assessments.
  • Coordinate assessment clinics and prioritize appointments, adapting to the unique needs of each individual.
  • Develop and implement effective management plans, follow-up care, and referrals to specialist services.
  • Provide support and guidance to junior team members, promoting a culture of collaboration and learning.
Working for Our Organisation

The Access Team operates Monday to Friday, 9am to 5pm, and works closely with Healthy Minds, The Transition Team, and other internal and external providers.

Staff are expected to participate in the overall development of the service, ensuring Clinical Governance principles underpin all developments.

Key Responsibilities
  • Participate in the duty worker rotation.
  • Contribute to the triage process of new referrals.
  • Carry out thorough assessments of service users' mental health problems and individual needs.
  • Develop individualized packages of care, including signposting where necessary.
  • Liaise with other professions and agencies to ensure the best possible service is provided.
Person Specification

We are seeking a skilled and experienced mental health professional who can demonstrate:

  • Significant post-registration experience in mental health.
  • Proven experience of working with clients with complex needs.
  • Experience of working in multi-disciplinary teams.
  • Ability to provide supervision to junior staff.

We value candidates with a strong understanding of current mental health legislation, NICE Guidance, and psychosocial assessment and risk assessment/management processes.

Excellent communication and IT skills are essential, as is the ability to form productive working relationships with service users, carers, and multi-disciplinary staff.
