About the Role

We are seeking a highly skilled and motivated Mental Health Practitioner to join our Older Peoples Mental Health Liaison Team in Stockport. As a key member of our team, you will work closely with the Urgent Community Response Team to provide specialist mental health nursing advice and support to individuals in crisis.

Main Responsibilities
  • Represent the Mental Health Liaison Service while working with the Urgent Community Response Team in Stockport.
  • Provide direct care to service users, including assessment, risk management, and psychological support.
  • Collaborate with multi-disciplinary teams to develop and implement effective care plans.
  • Assess and manage clients with co-morbidities of physical and mental health needs.
  • Model best practices in communication and care delivery to the Urgent Community Response Team.

Requirements
  • Registered Mental Health Nurse (RMN) qualification.
  • Experience of working in older peoples mental health inpatient, community, or liaison settings.
  • Ability to work autonomously and as part of a multi-disciplinary team.
  • Excellent communication and interpersonal skills.
