About the Role

We are seeking a highly skilled and compassionate Social Worker to join our Adult Services Learning Disabilities and Autism Community Teams in Lancashire. As a key member of our team, you will play a vital role in supporting individuals with learning disabilities and/or autism to live fulfilling lives.

Key Responsibilities
  • Conduct assessments and reviews of care arrangements to ensure individuals receive the best possible support.
  • Offer advice and information to individuals, families, and other stakeholders to promote independence and well-being.
  • Develop and implement personalized support plans to meet the unique needs of each individual.
  • Collaborate with other professionals and agencies to ensure seamless service delivery and effective outcomes.
  • Provide emotional support and guidance to individuals and their families, promoting a culture of empathy and understanding.
Requirements
  • Registered with Social Work England (SWE) or have applied to register.
  • Proven experience in working with individuals with learning disabilities and/or autism.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with individuals, families, and colleagues.
  • Ability to work independently and as part of a team, with a flexible approach to meet the needs of the service.
  • Strong problem-solving and analytical skills, with a focus on promoting independence and well-being.
