About the Opportunity

We are looking for a skilled and dedicated mental health practitioner to join our Early Intervention in Psychosis team as a Care Coordinator. As a Care Coordinator, you will work closely with service users, their families, and other healthcare professionals to deliver high-quality care and support.

The successful candidate will have experience in a relevant specialty, a strong understanding of the Mental Health Act and other relevant legislation, and excellent interpersonal skills. They will also be able to work independently, prioritize tasks effectively, and demonstrate a commitment to raising standards in clinical areas.

Key Responsibilities and Requirements

  1. To coordinate care plans for service users, ensuring that their assessed needs are met.
  2. To conduct assessments and develop risk management strategies to minimize risks.
  3. To communicate effectively with the multidisciplinary team and service users' families.
  4. To direct unqualified staff and ensure that they deliver high-standard care to service users.
  5. To stay updated on relevant legislation and best practices.

Benefits and Salary Information

This is a full-time position with a salary range of £37,338 - £44,962 per annum, paid pro-rata for part-time hours. Our comprehensive benefits package includes pension scheme, annual leave, and opportunities for training and professional development.
