Nurse Practitioner Specialist

We are seeking a skilled Registered Mental Health Nurse (RMN) or Registered Learning Disability Nurse (RNLD) to join our team at Hertfordshire Partnership University NHS Foundation Trust. As an Advanced Community Nurse Practitioner, you will play a vital role in delivering high-quality care to individuals with learning disabilities.
Your primary responsibilities will include assessing healthcare needs, implementing interventions, and collaborating with an interdisciplinary team to devise care plans and evaluate outcomes.
Additionally, you will support health promotion for people with learning disabilities, ensuring inclusion in mainstream services according to national and local policy and guidance.
Requirements- Professional registration with the Nursing and Midwifery Council (NMC).
- Compassion, resilience, and dedication to empowering service user independence.
- Effective communication skills, using various methods, with service users, other team members, carers/family, and agencies involved in the service user's care.
- A good understanding of legislation and national guidance related to the care and support of people with learning disabilities.
