Geriatric Care Management Specialist

We are seeking a skilled and compassionate Registered Nurse with Extended Class: RN(EC) privileges to join our team as a Geriatric Care Management Specialist. In this role, you will be responsible for providing high-quality patient care and leading our team of healthcare professionals in delivering exceptional service.
Key Responsibilities:
• Provide medical coverage to residents
• Consult with healthcare professionals to develop and implement care plans
• Promote evidence-based primary health care services
• Perform management tasks such as guiding policy development and approving procedures
Requirements:
• Registration with the College of Nurses of Ontario (CNO)
• Current certification in CPR/BLS
• Strong leadership and communication skills
