Job Summary

We are seeking a highly motivated Transitional Care Manager to join our Home Health Sales team at BAYADA Home Health Care. This role offers opportunities for professional growth and development in the healthcare sales and marketing industry.


About the Role

This hybrid position involves collaborating with area sales teams, processing referrals, and working from home 50% of the time, with travel to hospitals in NJ the remaining 50%. The successful candidate will have excellent communication and interpersonal skills, as well as experience in healthcare.


Responsibilities
  • Communicate BAYADA's core values and build relationships with referral sources and healthcare professionals.
  • Review documentation and coordinate care transitions with referral sources and physicians.
  • Collaborate with Transitional Care Managers, Marketing Managers, and Directors on key accounts.
  • Participate in sales meetings and demonstrate solid performance in key job dimensions.

Requirements

The ideal candidate will possess a Bachelor's degree (or equivalent experience), strong interpersonal and communication skills, and experience in the healthcare industry. A competitive salary of $65,000 - $85,000 per year, depending on experience, is offered.
