**Job Summary**

We are seeking a skilled and passionate Youth Transition Specialist to join our team at Derbyshire County Council. As a key member of our Transformation and Partnership Team, you will play a vital role in supporting young people as they transition into adulthood.

**Key Responsibilities**

  • Conduct strengths-based assessments and reviews to identify the needs of young people and develop personalized support plans.
  • Collaborate with colleagues across Adult Social Care, Children's Services, and other partner agencies to achieve positive outcomes for individuals.
  • Develop and maintain relationships with young people, their families, and other stakeholders to ensure effective support and guidance.
  • Stay up-to-date with relevant legislation, policies, and best practices in adult social care and transition to adulthood.

**Requirements**

  • Experience working in adult social care, preferably in a transition to adulthood role.
  • Knowledge of the Care Act 2014 and the Health and Care Act 2022.
  • Ability to work collaboratively with colleagues and external partners.
  • Excellent communication and interpersonal skills.
