Job Title: Social Worker

Derbyshire County Council is seeking a qualified social worker to join our Transformation and Partnership Team. As a key member of our team, you will play a vital role in supporting young people transitioning to adulthood.

About the Role

This is a full-time post for a fixed term of up to 2 years or internal secondment. You will work closely with colleagues across Adult Social Care, Children's Services, and other partner agencies to achieve positive outcomes for individuals.

Key Responsibilities
  • Completing strengths-based timely assessments, reviews, and support planning
  • Working collaboratively with colleagues to achieve independent outcomes for individuals
  • Developing and implementing person-centered, outcome-focused approaches
Requirements

We are looking for an experienced social worker with knowledge of the Care Act 2014 and The Health and Care Act 2022. A good understanding of the 'six themes of making it real' embedded within Think Local Act Personal (TLAP) is highly desirable. You will have excellent communication skills and be able to travel county-wide.
