Care Act Assessment Specialist

Job Summary:
We are seeking a skilled and motivated Social Worker to strengthen our Adult Social Care Transformation team. As a key member of the team, you will play a crucial role in reviewing and reassessing care packages for individuals with learning disabilities and autism, ensuring they receive the best possible support.
Key Responsibilities:
- Undertake Care Act assessments to review the need for one-to-one or two-to-one support, engaging with support staff, colleagues, family members, and providers to ensure support arrangements match individuals' identified outcomes.
- Review and reassess care packages for individuals living in Specialist Residential/Nursing Placements, both in and outside of the county, to ensure levels of support are appropriate and least restrictive.
- Be involved in reviewing and reassessing packages of care for individuals with learning disabilities and autism living in High Cost Supported Living/Extra Care Schemes, ensuring levels of support are appropriate and promote progression and development.
- Reduce the level of support commissioned in Specialist Residential/Nursing Placements as a result of reassessment of eligible needs, reducing over-reliance on formal support.
- Ensure timely applications for Continuing Health Care and provide support around DST reviews.
Team and Management:
You will be part of a dynamic team, working closely with a project manager and contract manager who will oversee program management, provider relationships, and market shaping sustainability.
As a member of the Adult Social Care Transformation team, you will be line-managed and supported in your role.
Requirements:
The successful candidate will be willing and able to travel when required, as the role may involve reviewing out-of-county placements.
