Job Overview

An exciting opportunity has arisen for a highly skilled and experienced Specialist Clinical Pharmacist to join the pulmonary rehabilitation team in Swansea Bay University Health Board. As a key member of the multidisciplinary team, you will work closely with healthcare professionals to deliver patient-focused care in various settings, including community locations, patients' homes, and outpatient settings.

Main Responsibilities
  • Manage a complex clinical caseload, providing specialist nutrition and dietetic advice to the Pulmonary Rehabilitation Team, utilizing highly developed communication skills based on negotiation, counseling, and behavioral change therapy.
  • Work independently to plan, implement, and evaluate evidence-based Nutrition and Dietetic interventions.
  • Provide expert advice in Pulmonary Rehabilitation within Primary Care, including nutrition support and weight management for a wide range of respiratory outpatients and at home, liaising with Respiratory consultants and specialist Respiratory Nurses regarding nutritional health and adequacy for Pulmonary Rehabilitation patients.
Requirements
  • B.Sc. Nutrition and Dietetics or equivalent qualification.
  • Post-registration training in a relevant area.
  • HCPC Registered.
Desirable Criteria
  • Recognized management/leadership qualification.
  • Recognized teaching qualification.
  • Student clinical educators course.
