Specialist Clinical Pharmacist

Job Overview
We are seeking a highly motivated and experienced Dietitian to join our Pulmonary Rehabilitation team in Swansea Bay University Health Board. As a Specialist Clinical Pharmacist, you will work alongside a multidisciplinary team to deliver patient-focused care in the community, including community locations, patients' own homes, and outpatient settings.
The post holder will be responsible for managing a complex clinical caseload, providing specialist nutrition and dietetic advice to the Pulmonary Rehabilitation Team. You will work independently with patients using highly developed communication skills based on negotiation, counselling, and behavioural change therapy.
Main Duties of the Job
- Management of own complex clinical caseload providing specialist nutrition and dietetic advice in specialty field to Pulmonary Rehabilitation Team.
- Working independently using highly developed communication skills planning, implementing, and evaluating evidence-based Nutrition and Dietetic interventions.
- Providing expert advice in Pulmonary Rehabilitation within Primary Care, e.g., Nutrition support and weight management for a wide range of respiratory outpatients and at home.

Person Specification
- B.Sc. Nutrition and Dietetics or equivalent.
- Post-registration training in a relevant area.
- HCPC Registered.
Knowledge
- MSc or its equivalent level of knowledge + advanced postgraduate training/experience in relevant areas.
- Working knowledge and understanding of professional ethics and their application in practice.
Experience
- Experience of working as a Specialist Dietitian in Nutrition and Dietetics within specialist area.
- Experience of a high level of specialist knowledge of service delivery in a variety of different settings.
- Experience of supervising/mentoring staff.
Skills
- Developed counselling skills to use with patients/clients, carers with highly complex needs Clinical Supervision skills.
- Group presentation skills and ability to work effectively single-handed with individuals.
- Anthropometric measurement skills using skinfold calipers and hand dynamometry.
