Job Overview

An exciting opportunity has arisen for a highly skilled and experienced Specialist Clinical Pharmacist to join the pulmonary rehabilitation team in Swansea Bay University Health Board. As a key member of the multidisciplinary team, you will work closely with healthcare professionals to deliver patient-focused care in various settings, including community locations, patients' homes, and outpatient settings.

Main Responsibilities
  • Manage a complex clinical caseload, providing specialist nutrition and dietetic advice to the Pulmonary Rehabilitation Team, using highly developed communication skills based on negotiation, counselling, and behavioural change therapy.
  • Work independently to plan, implement, and evaluate evidence-based Nutrition and Dietetic interventions, utilizing highly developed communication skills.
  • Provide expert advice in Pulmonary Rehabilitation within Primary Care, including nutrition support and weight management for a wide range of respiratory outpatients and at home, liaising with Respiratory consultants and specialist Respiratory Nurses regarding nutritional health and adequacy for Pulmonary Rehabilitation patients.

Person Specification

Essential Criteria

  • B.Sc. Nutrition and Dietetics or equivalent.
  • Post-registration training in a relevant area.
  • HCPC Registered.

Desirable Criteria

  • Recognised management/leadership qualification.
  • Recognised teaching qualification.
  • Student clinical educators course.

Knowledge

Essential Criteria

  • MSc or its equivalent level of knowledge + advanced postgraduate training/experience in relevant areas.
  • Evidence of a wide variety of clinical knowledge.
  • Working knowledge and understanding of professional ethics and their application in practice.

Desirable Criteria

  • Knowledge/experience working within pulmonary rehabilitation.

Experience

Essential Criteria

  • Experience of working as a Specialist Dietitian in Nutrition and Dietetics within a specialist area.
  • Experience of a high level of specialist knowledge of service delivery in a variety of different settings.
  • Experience of specialist knowledge of respiratory medicine and rehabilitation.
  • Experience of supervising/mentoring staff.

Desirable Criteria

  • Experience of policy/service development.
  • Experience of strategic planning process.
  • Membership of Special Interest Group of British Dietetic Association.

Skills

Essential Criteria

  • Developed counselling skills to use with patients/clients, carers with highly complex needs, Clinical Supervision skills.
  • Group presentation skills and ability to work effectively single-handed with individuals.
  • Anthropometric measurement skills using skinfold calipers and hand dynamometry.
  • Developed behaviour change/motivational interviewing skills.

Desirable Criteria

  • Ability to interpret Bioimpedance Analysis appropriately.
  • Ability to speak Welsh.
