Critical Care Outreach Specialist

We are seeking a skilled Critical Care Outreach Specialist to join our team at Tameside and Glossop Integrated Care NHS Foundation Trust. This permanent role offers an exciting opportunity for a registered nurse with recent experience in critical care or an acute/emergency care background.
As a key member of our Critical Care Outreach Team, you will play a vital role in facilitating discharges from Critical Care to wards, providing clinical training to ward staff, and acting as a role model and mentor for ward staff. You will also undertake follow-up of Critical Care patients post-discharge from Intensive Care.
The successful candidate will have excellent clinical reasoning and decision-making skills, as well as strong leadership skills and a clear vision for the scope of the outreach service. If you have experience in managing acutely unwell patients and a proven track record in delivering clinical teaching sessions, we encourage you to apply.
Responsibilities- Facilitate discharges from Critical Care to wards
