Mental Health Specialist Practitioner

Job Summary:
As a Registered Mental Health Nurse, you will be an integral part of the multi-disciplinary memory assessment service team. Your role will involve ensuring the effective assessment, planning, consultation, and formulation towards diagnosis.
Key Responsibilities:
- Develop a specialist knowledge and understanding of strategies and interventions in the treatment of dementia.
- Referring to other agencies within the service or referring to other services or agencies where necessary.
- Confidently identifying risk factors and taking appropriate action.
- Providing a communication structure between GPs and other services in LCFT, non-statutory services, to ensure the highest possible standards of patient care.
- Offering mentoring and clinical supervision to other staff and trainees on placement.
- Actively promoting the Trust values within the MAS service and in accordance with the NMC and HPC code of conduct.
- Assisting the team manager in ensuring that leadership within the MAS team provides effective assessment, formulation, planning, and monitoring of care given to patients and their carers.
