Mental Health Practitioner

This is an exciting opportunity for a skilled Mental Health Practitioner to join our Home Treatment Team in Lancaster, providing intensive support to individuals experiencing acute episodes of mental illness or emotional distress.
As a key member of the multi-disciplinary team, you will work closely with Mental Health Nurses, Support Workers, Occupational Therapists, Psychologists, and Consultant Psychiatrists to deliver high-quality care and promote recovery.
The successful candidate will possess excellent communication skills, able to build strong relationships with service users, their families, and referrers from various healthcare services.
Key Responsibilities- Provide comprehensive assessments and triage services, signposting or facilitating onward referral to mental health pathways as needed.
- Develop and implement individualized care plans, working collaboratively with the multidisciplinary team to ensure seamless transitions and continuity of care.
- Collaborate with local healthcare services, including IRS, Police, and Acute Hospital colleagues, to facilitate effective referrals and reduce unnecessary admissions.
To be successful in this role, you must have:
- A registered qualification as a Mental Health Nurse, Occupational Therapist, or Social Worker.
- Experience working with individuals experiencing acute mental health issues, including crisis planning and contingency planning.
- Demonstrated knowledge of care pathways for psychosis and non-psychosis, as well as relevant interventions.
