Community Care Navigator

Raised Awareness:
As a Carers Development Worker at LCVS, you will be instrumental in raising awareness about the importance of supporting carers in Liverpool. This role offers a unique opportunity to make a tangible difference in the lives of those who dedicate themselves to caring for others.
Your primary responsibility will be to work closely with carers to develop comprehensive support plans, which will involve providing information about available services and facilitating connections with local authorities.

Key Accountabilities:
- Schedule Home Visits to conduct assessments and gather essential information from carers.
- Collaborate with Local Authorities to ensure effective delivery of support services.
- Implement and Monitor Care Plans to guarantee the best possible outcomes for carers.
- Document Progress and maintain accurate records to inform future decisions.
- Bid farewell to traditional working models and embrace innovative approaches to support carers effectively.
