Job Summary

We are seeking a dedicated individual to provide high-quality care and support to adults with autism, dementia, learning disabilities, mental health conditions, and challenging behaviors. This role involves building life skills, achieving goals at the individual's own pace, and promoting independence.

Responsibilities
  • Support individuals to develop their life skills and achieve personal goals.
  • Supervise colleagues, lead team meetings, and assist with performance development.
  • Lead support planning and act as point of contact during site leadership's absence.
  • Accompany individuals to medical appointments and monitor physical and emotional well-being.
  • Establish and maintain supportive relationships with family members and communicate effectively with specialists.
  • Assist with daily living tasks such as washing, dressing, and food preparation.
  • Maintain knowledge of policies and procedures and demonstrate competence in verbal and written English.
Requirements

Able to work x3 days per week (07:45 - 20:15), including weekends.
