**Job Title:** Disability Support Specialist
We are seeking an experienced professional to join our team as a Disability Support Specialist in the Young Adults Transitions Service. This is an exciting opportunity to work with young people aged 14-25 who have additional needs, supporting them to develop skills and achieve their goals.
Salary: £35,000 - £42,000 per annum
About the RoleWe are committed to providing high-quality support to young people with additional needs. Our multi-disciplinary team works closely with young people, their families, and partner agencies to deliver a range of services.
- Supporting young people to plan for important life changes and make informed decisions about their future.
- Managing risks by thinking creatively about safe solutions while considering everything that is important to the young person.
- Linking with local community groups and working in partnership to provide opportunities for our young people to work and learn.
The Young Adults Transitions Service is a citywide team who can work with young people with additional needs from age 14-25. We complete Care Act assessments and provide occupational therapy support to maximise independent living.
Key Responsibilities- Working with young people, their families, and partner agencies to deliver a range of services.
- Completing Care Act assessments and providing occupational therapy support.
- Developing and implementing support plans to meet the needs of our young people.
- Previous experience working with young adults or transitions services.
- Able to travel across the City.
- Excellent professional support, including opportunities for development and learning.
- Good quality supervision and regular feedback.
- Opportunities for career progression and leadership development.
