About the Role

We are seeking a highly skilled and experienced Mental Health Practitioner to join our Access Team at Pennine Care NHS Foundation Trust. As a key member of our team, you will play a vital role in providing high-quality care and support to individuals with mental health needs.

Key Responsibilities
  • Conduct comprehensive assessments of individuals' mental health needs, including risk assessments.
  • Develop and implement individualized care plans, including referrals to specialist services.
  • Provide education and support to patients, families, and carers.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination.
  • Participate in audit and quality improvement activities to enhance service delivery.
Requirements
  • Postgraduate qualification in a relevant field (e.g., mental health nursing, social work, psychology).
  • Experience working with individuals with mental health needs, including those in crisis.
  • Strong assessment and intervention skills, with a focus on psychosocial approaches.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Ability to work autonomously and as part of a team, with a flexible and adaptable approach.
Desirable Qualifications and Experience
  • Non-medical prescribing qualification.
  • Experience working in a primary care mental health setting.
  • Knowledge of NICE guidelines and current mental health legislation.
