We are seeking a motivated and experienced Occupational Therapist to join our Frailty and Admission Avoidance team. The ideal candidate will have a strong background in occupational therapy, excellent communication skills, and the ability to work effectively in a fast-paced environment.

Job Description:

Main Duties:
  1. To assess and treat patients with acute health needs, working closely with the multidisciplinary team to ensure seamless care.
  2. To develop and implement effective care plans, including history taking, physical assessment, and treatment planning.
  3. To work collaboratively with other healthcare professionals to prevent hospital admissions and promote patient independence.

Requirements:

  • Qualified Occupational Therapist with HCPC registration
  • Experience working with people living with frailty
  • Good knowledge of health and safety and risk management

Salary: £37,338 to £44,962 per annum based on full-time hours.
