Job Title: Clinical Care Specialist

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is a leading healthcare provider, offering a wide range of services across Hampshire and the Isle of Wight. We are seeking an experienced Clinical Care Specialist to join our team in Urgent Community Response and Frailty Hospital at Home.

About the Role:
The post holder will work under supervision to develop high-level clinical skills and autonomy, managing complex care in partnership with individuals, families, and carers. This will involve analysis and synthesis of problems across various settings, enabling innovative solutions to enhance patient experience and improve outcomes.

Main Duties:
Assessment, diagnostics, and treatment of acutely unwell patients at risk of hospital admission. Supervision and development from qualified advanced clinical practitioners and consultant practitioners.

Requirements:
* Appropriate professional qualification for registration with the NMC or HCPC.* Professional UK registration with the NMC or HCPC.* Degree level award plus postgraduate specialist training, experience, or equivalent level qualifications.* Good command of English language written and verbal.* Computer literacy in data entry and IT communication.* Independent non-medical prescribing or pharmacology level 7 profession relevant qualification (desirable).* Evidence of developing specialized clinical skills across frailty, urgent response, and hospital at home care.* System working with individuals, families, carers, communities, and others for population health and reducing inequalities.* Contribution to service development using quality improvement methodology.* Awareness of local and national agendas across area of specialty.

Benefits:
