Home-Based Care Specialist

We are seeking a skilled Extended Scope Clinical Practitioner (ECP) to join our team at the Mid Hampshire Urgent Community Response and Frailty Hospital at Home Virtual Ward. As an ECP, you will work closely with patients and families to provide high-quality care in their own homes.

Our team focuses on providing hospital-level interventions and diagnostics as an alternative to hospital admission for patients with acute needs. You will be part of a multidisciplinary team that includes consultant practitioners, nurses, and other healthcare professionals.

We aim to provide seamless care across the region by integrating services from Southern Health, Solent NHS Trust, Isle of Wight NHS Trust's community and mental health teams, and Hampshire CAMHS.

Key Responsibilities
  • Work collaboratively with a consultant practitioner-led team to assess, diagnose, and treat patients in their homes.
  • Deliver high-quality care that meets the needs of patients with complex conditions.
Person Specification
  • Appropriate professional qualification allowing registration with the NMC or HCPC.
  • Professional UK registration (e.g., NMC/HCPC).
  • Degree-level award.
  • Independent non-medical prescribing or pharmacology level 7 profession-relevant qualification.
Salary Information
