About the Role
We are seeking a skilled and compassionate Mental Health Specialist to join our team at Derbyshire Healthcare NHS Foundation Trust. As an Older Adult Community Support Worker, you will play a vital role in supporting individuals with functional mental health conditions in a community setting.
Main Responsibilities
The successful candidate will be responsible for conducting comprehensive person-centred psychosocial assessments, risk assessments, and formulating presenting mental health needs. They will build therapeutic relationships, develop care plans, and empower individuals to take control of their health.
Key Requirements
- Relevant qualification in nursing or occupational therapy
- Post-qualification experience in working with older adults with functional mental health diagnoses
- Experience in community working and linking with partner agencies
