Job Title: Social Worker

We are seeking a skilled and motivated Social Worker to join our Adult Social Care Prevention and Personalisation team in the Derbyshire Dales South Team based at The Leys in Ashbourne.

About the Role

As a Social Worker, you will be responsible for undertaking Care Act assessments, support planning, reviews, and safeguarding enquiries. You will work closely with reablement care staff, Occupational Therapy staff, and other health colleagues in a multi-disciplinary model.

Key Responsibilities
  • Undertake Care Act assessments and support planning to ensure that support arrangements match a person's identified outcomes.
  • Conduct safeguarding enquiries and complete mental capacity and best interest assessments.
  • Work collaboratively with other teams and professionals to achieve positive outcomes for service users.
  • Develop and maintain effective relationships with service users, their families, and carers.
